- This form records a nominator's attestation supporting a person's application for a National Innovation (subclass 858) visa, confirming that the applicant has an internationally recognised record of exceptional achievement in their area of talent and would be an asset to the Australian community. It is lodged together with the visa application as part of the assessment.

- A nominator, being an individual (an Australian citizen, permanent resident or eligible New Zealand citizen) or an Australian organisation acting to attest to the applicant's standing. It is not completed by the visa applicant.
