- This form allows a person holding an Extended Eligibility (Dependent Child, subclass 445) visa to be added to a parent's permanent partner visa application. It provides the pathway for that child, and any of their own dependants, to be included so the permanent partner outcome can extend to them.

- The subclass 445 dependent child visa holder (or a person acting on their behalf where the child is a minor), in connection with a parent's existing partner visa process.
- Information it collects (in general terms)
- General categories include the child applicant's personal and identity details, passport and travel information, details of any dependants of the child, links to the relevant parent's partner visa, and declarations relating to Australian values, health and character.
- When it is used
- Used where a parent has been granted a provisional or temporary partner visa but not yet the permanent partner visa, and their subclass 445 dependent child needs to be added to that permanent application to avoid losing their permanent visa pathway.
