What it is for
A form for telling the Department that a change in the person's circumstances now affects an answer they previously gave in a visa application. It fulfils the notification obligation under section 104 of the Migration Act 1958.
Who completes it
The visa applicant or visa holder whose circumstances have changed.
Information it collects (in general terms)
General categories include the person's identity, passport and contact details, relationship status, and consent to electronic communication, together with details of the changed circumstances and how they affect earlier answers.
When it is used
Used when something about the applicant's or holder's situation changes so that an answer given on a lodged application (or during a matter such as a proposed visa cancellation) is no longer accurate. A separate form is used to correct information that was incorrect when given rather than changed later.
