- What it is for
- A form for telling the Department that a change in the person's circumstances now affects an answer they previously gave in a visa application. It fulfils the notification obligation under section 104 of the Migration Act 1958.
- Who completes it
- The visa applicant or visa holder whose circumstances have changed.
- Information it collects (in general terms)
- General categories include the person's identity, passport and contact details, relationship status, and consent to electronic communication, together with details of the changed circumstances and how they affect earlier answers.
- When it is used
- Used when something about the applicant's or holder's situation changes so that an answer given on a lodged application (or during a matter such as a proposed visa cancellation) is no longer accurate. A separate form is used to correct information that was incorrect when given rather than changed later.
