- What it is for
- This form is used to apply to become a sponsor for an applicant for a Visitor visa (subclass 600) in the Sponsored Family stream, and to record the sponsor's formal undertaking of responsibility.
- Who completes it
- The proposed sponsor, who must be a settled Australian citizen or permanent resident aged 18 or over and, in most cases, a relative of the visitor (or another eligible person such as a parliamentarian or government representative).
- Information it collects (in general terms)
- General categories of information about the sponsor and their eligibility, the relationship to each visitor being sponsored, supporting evidence of that relationship and of the sponsor's citizenship or residence status, and a signed sponsorship undertaking accepting financial and compliance obligations.
- When it is used
- Used when a person offers to sponsor a family member's Visitor visa (subclass 600) application in the Sponsored Family stream, or when the Department has requested sponsorship for a Tourist stream applicant; the completed form is attached to each visitor's online application.
