- This form is used to request a Certification of Immigration Status from the Department confirming that a person's Temporary Resident visa has ceased to be in effect and that they have left Australia, for use in making a Departing Australia Superannuation Payment (DASP) claim.

- The former temporary resident seeking the certification. Each person requesting a certification completes a separate form, and a fee applies to each request.
