What it is for
This form provides an endorsed identity declaration used to verify the identity of a person applying online for Australian citizenship by descent or evidence of citizenship, which cannot be approved where identity cannot be verified.
Who completes it
The citizenship applicant completes their own sections, and a qualifying endorser (an Australian citizen, or a citizen of the country of residence when applying from overseas) in an approved profession who has known the applicant for the required period and is not related to them completes the remaining sections and endorses the photograph.
Information it collects (in general terms)
General categories of information: the applicant's identifying details, and the endorser's identity, occupation and declaration, together with an endorsed passport-sized photograph of the applicant meeting set photographic requirements.
When it is used
Used when a person applies online for Australian citizenship by descent or for evidence of citizenship and must supply an independently endorsed identity declaration and photograph to support identity verification.
