What it is for
This is a supplementary form used to provide extra personal, travel and background information in support of a visa application. It captures detail beyond what the primary application form records.
Who completes it
The visa applicant; it is to be completed by all applicants aged 16 years or over.
Information it collects (in general terms)
It gathers personal and identity details, contact and address information, passport and travel-document particulars, prior visa and immigration history, purpose and dates of travel, details of travelling dependants or family members, and current employment information.
When it is used
It is used to accompany and support a visa application when the Department requires additional personal particulars from an applicant.
