- This form records a sponsorship from an authorised state or territory government agency that is required for the Investor Retirement visa. It is submitted together with the main retiree visa application to evidence that sponsorship.

- The visa applicant completes their own details in Part A, and an authorised officer of the sponsoring state or territory government agency completes the sponsorship section in Part B.

- It gathers the applicant's and partner's personal and contact details, a signed applicant declaration and consent, and the sponsoring government agency's details confirming the sponsorship.
- When it is used
- It is used when applying for an Investor Retirement visa and must be lodged, signed and stamped, together with the temporary retiree visa application (form 1383) within the validity period of the sponsorship.
