- What it is for
- This form supports the Department in considering care arrangements for a minor under the Unaccompanied Humanitarian Minor Program and helps assess the minor's status under the Immigration (Guardianship of Children) Act 1946. It is expressly not evidence of an established care arrangement and is not a legal document.
- Who completes it
- The proposed carer - a person over 21 who intends to provide care for the minor - completes and signs the care undertaking.
- Information it collects (in general terms)
- It gathers the minor's identity, birth and citizenship details, biological parents' details, and for the proposed carer their identity, relationship to the minor, household and living arrangements, contact details, and undertakings about schooling, daily needs and financial support.
- When it is used
- It is used within the humanitarian program when the Department is considering who might care for an unaccompanied minor and the proposed carer sets out their circumstances and willingness to provide care.
