What it is for
This form provides the written Australian Values Statement that certain temporary visa applicants complete to confirm they understand and will uphold the shared values of Australian society and obey Australian law. It supplements a visa application when the applicant has not already made the same declaration on the main application form.
Who completes it
The temporary visa applicant, together with each accompanying person aged 18 or over included in the application who is directed to complete it.
Information it collects (in general terms)
The signatory's name and dated signature, alongside a set of pre-printed declarations and acknowledgements about Australian values that the person is affirming; an office-use section records departmental reference details.
When it is used
Used only when a temporary visa applicant is specifically advised on their visa application form to complete a values statement and has not already signed one on that application.
