What it is for
This form is used to apply to become an Australian citizen again after having previously ceased to be one, under the Australian Citizenship Act 2007. It covers people who lost citizenship in specified historical circumstances, and citizenship recommences on the date the application is approved.
Who completes it
A former Australian citizen seeking to resume citizenship. Children under 16 can be included on a responsible parent's application, and applications for those children must be made by a responsible parent.
Information it collects (in general terms)
General categories include personal and identity details, evidence of the applicant's date of birth, birth name and changes of name, current residential address, good-character documents (including overseas penal clearance certificates where relevant), an identity declaration by an authorised person, and certified copies of supporting documents.
When it is used
Used by people who ceased to be Australian citizens in particular ways, such as by acquiring another country's citizenship before 4 April 2002, by renouncing to retain or acquire another citizenship or avoid significant hardship, or as a child whose parents ceased to be citizens. Payment is made through ImmiAccount before the form is posted to the Department.
