- A standardised script and record for conducting a biometric identification test (photograph and fingerprints) on an adult in immigration detention under the Migration Act 1958. It ensures the required information is read to the person and their understanding is documented.
- Who completes it
- Administered and completed by the authorised officer conducting the test and the assisting officer; the adult detainee responds to the scripted statements. It is attached to the detainee's file rather than lodged by an applicant.

- Detainee identity and date of birth, client identifier, test date and time, interpreter and same-sex-officer preferences, whether an independent person is present, the detainee's recorded understanding of each scripted statement, and officer declarations and signatures.
