- Administered and completed by the authorised officer conducting the test and the assisting officer, with a parent, guardian or independent person representing the detainee; it is attached to the detainee's file rather than lodged by an applicant.

- Detainee identity and date of birth, client identifier, details of the accompanying parent/guardian/independent person, test date and time, interpreter and same-sex-officer preferences, the recorded understanding of each scripted statement, and officer declarations and signatures.
