- This form is used to add a further person to a visa application that has already been lodged but not yet decided, where the relevant visa allows applicants to be added after lodgement. It captures the new person's details and supports any applicable charge.
- Who completes it
- The main applicant on the original visa application (or a representative acting for them) submits it to add a dependent child or partner.
- Information it collects (in general terms)
- Details of the original application and main applicant, the additional applicant's identity, passport, nationality, birth and contact information, their relationship to the main applicant, national identity documents, and general health-related declarations, together with payment information for any charge.
- When it is used
- Used after a visa application has been lodged and before a decision is made, when an eligible additional applicant (typically a partner or dependent child) is to be included in that existing application.
