What it is for
This form sets out the standards of conduct expected of certain adults being considered for a Subclass 050 Bridging E visa and is signed as an acknowledgement of, and agreement to, that code of behaviour while living in the Australian community.
Who completes it
The bridging visa applicant signs it (an adult affected by the code), with an eligible witness verifying identity and witnessing the signature; the signed form is then returned to the Department.
Information it collects (in general terms)
The signer's address and identity details, their signature and date confirming agreement to the code, and the eligible witness's details and confirmation of identity verification.
When it is used
Completed before an eligible adult can be considered for the grant of a Bridging E visa, including where a previously granted bridging visa has ceased or is about to cease and a new one is sought.
