- This form sets out the standards of conduct expected of certain adults being considered for a Subclass 050 Bridging E visa and is signed as an acknowledgement of, and agreement to, that code of behaviour while living in the Australian community.
- Who completes it
- The bridging visa applicant signs it (an adult affected by the code), with an eligible witness verifying identity and witnessing the signature; the signed form is then returned to the Department.

- The signer's address and identity details, their signature and date confirming agreement to the code, and the eligible witness's details and confirmation of identity verification.
- When it is used
- Completed before an eligible adult can be considered for the grant of a Bridging E visa, including where a previously granted bridging visa has ceased or is about to cease and a new one is sought.
