- This form records a person's acknowledgement that their Status Resolution Support Services (SRSS) provider has given them an orientation on emergency and fire safety procedures and advice on safe gardening practices.
- Who completes it
- The individual (and their family where applicable) receiving SRSS support, with the SRSS provider signing as witness.
- Information it collects (in general terms)
- The individual's identifying details and signed acknowledgement that the safety orientation was provided, together with witness and, where used, interpreter details.
- When it is used
- Used within the SRSS Programme after a provider has delivered home safety and gardening safety orientation, to document that the information was received and understood.
