What it is for
This is an information sheet explaining which family members may be included in certain visa applications and the general requirements and evidence involved. It sets out how partners, children (including newborn children), and, for some visa types, other relatives may be treated as members of the family unit.
Who completes it
This is guidance material rather than a lodged form; it is read by the primary visa applicant considering whether to include family members.

As an information sheet it gathers no applicant data itself; it explains general categories of information and evidence that may be needed elsewhere, such as relationship and dependency evidence, identity and birth documentation, and consent arrangements for children under 18.
