Consent to Collect or Disclose a Child's Personal Information

What it is for

This form records consent for the Department of Home Affairs, or another named agency or authority, to collect or disclose a child's personal information for purposes such as arranging health, education, care and welfare, or investigating possible harm to the child.

Who completes it

It is completed either by a parent, guardian, carer or custodian on the child's behalf, or by the relevant consenting party where no such representative is acting.

Information it collects (in general terms)

It gathers the child's identifying and contact details, the identifying details and relationship of any parent/guardian/carer/custodian acting for the child, and the scope of the information to be collected or disclosed along with the consenting person's signature.

When it is used

It is used where a child is in the Department's care or subject to detention or community arrangements, and consent is needed before their information is shared with welfare authorities, service providers, or other agencies.
