- This form supports a student affected by COVID-19 who is applying for a further Student (Subclass 500) visa or a Temporary Graduate (Subclass 485) visa, documenting the disruption to their studies so it can be considered as part of that visa application.

- It is completed and signed by the student's registered education provider, and the completed form is then included by the student with their online visa application.

- It gathers the education provider's registration and contact details, the student's identity and passport information, which visa is being applied for, course and enrolment details including deferral or online-study dates, and the provider's declaration.
