What it is for
This form supports a student affected by COVID-19 who is applying for a further Student (Subclass 500) visa or a Temporary Graduate (Subclass 485) visa, documenting the disruption to their studies so it can be considered as part of that visa application.

It is completed and signed by the student's registered education provider, and the completed form is then included by the student with their online visa application.
