- What it is for
- This form records student guardianship and welfare arrangements for a student visa applicant or holder who is under 18. It is used to nominate a guardian, confirm care arrangements with a parent, custodian or relative, or notify the Department of a change of guardian.
- Who completes it
- The nominating student (the student visa applicant or holder). A separate form must be completed for each student nominating the same person as a guardian.
- Information it collects (in general terms)
- General categories include the student's personal and identity details, the nature of the guardianship and welfare arrangements, details of the nominated guardian or relative and their relationship to the student, character-related information, and consent to electronic communication.
- When it is used
- Used when lodging a student visa application for a minor (with form 157A), when a guardian applies for a Student Guardian visa (with form 157G), or when an existing student visa holder wishes to change their welfare arrangements, in which case it is emailed to the nearest departmental office.
