- Captures information about a member of an applicant's family unit who is aged 18 or over, so the Department has details of each family member whether or not they intend to migrate.
- Who completes it
- The main visa applicant, who completes a separate copy for each qualifying family member; the family member signs the required values statement where applicable.
- Information it collects (in general terms)
- The main applicant's identifying details, and the family member's identity, names used, sex, date and place of birth, and citizenship information, together with a signed Australian-values statement.
- When it is used
- Used alongside a visa application when the applicant's family unit includes a child or other dependent relative aged 18 years or over, whether that person is migrating or not.
