- What it is for
- This is a short supporting form used to record details of an applicant's family members. It supplements a visa application (for example, a Visitor visa) by giving the department a picture of the applicant's immediate and extended family.
- Who completes it
- The visa applicant completes and signs the form; where another person assists in completing it, that assistance is acknowledged in the declaration, and a witness also signs.
- Information it collects (in general terms)
- General categories include, for each listed family member, their name, date of birth, relationship to the applicant and relationship status, home address, and whether they have previously visited Australia, together with a signed applicant declaration.
- When it is used
- Used as an attachment to a visa application when the department requires a consolidated summary of an applicant's family members (including half, step and adopted relatives), completed in English.
