What it is for

This is a departmental information-gathering form used to assess an applicant's background and character as part of visa or citizenship processing. It provides a detailed personal history to support integrity and character checks.

Who completes it

The applicant, who must be 16 years of age or over, completes the form in English when requested by the office processing their application.

General categories include personal and identity details and other names used, passport and travel-document history, national identity documents, address and contact history, and further background information used for character and identity assessment; it is completed in English.

When it is used

Used when the office processing a visa or citizenship application asks an applicant aged 16 or over to provide a comprehensive personal history to support assessment, including character assessment.
