- What it is for
- This form is a written agreement with the Australian Government in which a person commits to follow-up health monitoring or treatment after their visa is granted. It is typically required where a health examination indicated the need for post-arrival follow-up for conditions such as tuberculosis, hepatitis B or C, HIV, or leprosy, and it obliges the signer to attend an appointment with a health authority and undergo any treatment or investigation directed.
