What it is for
This form allows a person who knows the visa applicant and their partner or fiance(e) to provide a statement supporting a Partner or Prospective Marriage visa application. The Department uses such statements, among other evidence, to assess the social aspects of the claimed relationship.
Who completes it
A witness (a third party) who knows the couple and the history of their relationship and is at least 18 years of age.
Information it collects (in general terms)
General categories include the witness's identity and contact details, how they know the couple and the extent of their contact, their view on whether the relationship is genuine and continuing with reasons, and evidence of the witness's own identity and status.
When it is used
Used to accompany a Partner or Prospective Marriage visa application, where up to several supporting statements may be requested during processing to help establish the genuineness of the relationship.
